About the Program
Coastal’s Joint Health Program is part of the Musculoskeletal (MSK) Health Program, and is specifically designed to care for those Coastal patients with hip or knee osteoarthritis.
The program takes a long-term, team-based approach to helping patients receive the necessary support they need to manage their osteoarthritis of the hip or knee.
The team includes:
- your Coastal Medical primary care clinician
- your Coastal Medical musculoskeletal physician
- providers from Performance Physical Therapy
In addition, our Joint Health Program team includes Evolution Nutrition and Providence Behavioral Health, who are community partners who provide optional, yet valuable, services.

Together we ensure all aspects of your physical and mental well-being related to your hip or knee osteoarthritis are addressed:
- Exercise
- Nutrition and Weight Management
- Activity and Pain Education
- Social and Behavioral Health needs
Based on your needs, the Joint Health Team will create a care plan to match your health goals and preferences.
Continued Support
Once you are able to manage your condition, the Joint Health Team will offer follow-up care and resources to ensure your continued success.
